Registrations are OPEN for the upcoming 2019 baseball season!

Registration fees are $140 for one player, $250 for two players, $320 for three or more players (in the same family)*



Register BEFORE February 18 and fees are $120 for one player, $210 for two players, $260 for three or more players (in the same family)*

*$90 additional umpire fee for Junior and Senior divisions (ages 13-16 as of August 31)

Returning Players

1. If your son/daughter plays under a Regulation II(d) waiver because you moved in the past  you DO NOT have to provide proofs of residency.  The waiver grandfathers your son/daughter in Midway for as long as they play.

  • Wondering what waiver this is?  This waiver is for those who used to reside within Midway’s boundaries, moved outside the boundaries, but continue to play with Midway.  No proofs of residence will be required for these players. Not sure your son/daughter plays under a waiver?  Just ask Chris.


2. If your son/daughter has a school enrollment waiver on file you DO NOT have to provide proofs of residency.  Not sure?  Ask Chris.

3. If your son/daughter has a Tournament Player Verification (TPV) form on file you DO NOT have to provide proofs of residency.

Note that players who were a past member of the one of the following have a TPV on file.  These players will be grandfathered and will NOT need to provide new proofs of residency.

  1. 10 Year Old All-Star Team
  2. Major District All-Star Team
  3. Junior All-Star Team
  4. Senior All-Star Team

4. Original birth certificates will NOT be necessary UNLESS the league currently does not have a copy on file.

5. Please keep in mind that should your son/daughter make an all-star team for the FIRST time this year, an original birth certificate WILL BE REQUIRED in order for Midway to have the affidavits approved by District.  Don't worry, it will be returned to you.  Those players who have a TPV on file will not have to provide an original birth certificate.

Are you a returning player who has moved?

Good news!  You can still play with Midway if you played last year!  Please contact Chris Allen.  Chris will take care of all the paperwork.  All he will need is 3 proofs of residence from your OLD address when you lived in Midway's boundaries or the school enrollment form from last year if you played under the school waiver.  Chris will have these documents IF provided last year by the parent/guardian.  If they were not provided, then copies will need to be provided.  Please contact Chris and he can walk you through the steps and the required documentation.


Proofs of Residency

shall be established and supported by documents containing the full residence which includes parent(s) or guardian(s) name, street address, city, state and zip code information, dated or in force between February 1, 2018 (previous year) and February 1, 2019 (current year), from one or more documents from EACH of the three Groups outlined below:

Group I

1. Driver’s License (photo ID of parent(s) or guardian(s) with qualifying residence address)
2. School records (home address of player’s parents or guardians)
3. Vehicle records (i.e., registration, lease, etc.)
4. Employment records
5. Insurance documents (with residence address)

Group II

1. Welfare/child care records
2. Federal records (i.e., Federal Tax, Social Security, etc.)
3. State records
4. Local (municipal) records
5. Support payment records
6. Homeowner or tenant records
7. Military records

Group III

1. Voter’s Registration
2. Utility bills (i.e., - gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
3. Financial records (loan, credit, investments, etc.)
4. Medical records
5. Internet, cable, or satellite television records

NOTE: Example – Three documents from the same Group (utility bill, cable bill, and bank statement) constitute only ONE document.

Getting the proper proofs taken care of now is extremely important.  Otherwise, come all-star time and your son/daughter is named to an all-star team, parents will need to go back and provide acceptable documentation.  Acceptable means dated between February 1, 2018 and February 1, 2019 and meets the criteria outlined above.
